Rule out the medical stuff.

You have a family-context range. You know what can influence height. Now the biggest question: is there something medically going on that a workup would catch?

What a pediatric endocrinologist actually does.

A pediatric endocrinologist is a doctor who specializes in kids' hormones and growth. The evaluation is not scary. It is a conversation, a hand X-ray to check bone age, a blood draw to look at a few key markers, and sometimes a growth-hormone stimulation test if the first round of results points that way.

Most families walk out with reassurance and a plan. Some walk out with a diagnosis that changes the trajectory. Either way, you stop guessing.

When to ask your pediatrician for a referral.

  • Below the 3rd percentileEspecially if he has been dropping across percentile lines rather than tracking his own curve.
  • Growth velocity under 5cm/year past age 4Slow velocity is a stronger flag than a low percentile alone.
  • Crossing two percentile lines downwardThe change matters more than the number.
  • No puberty signs by age 14 in boysDelayed puberty is worth a specialist read.

You do not need to meet a threshold to ask. If you are worried, that is enough. Bring a printed growth chart. Ask specifically about a bone age X-ray and a referral to a pediatric endocrinologist.

What the workup usually includes.

The first visit is mostly a conversation and a plan. Common next steps: a bone age hand X-ray, a small blood panel (IGF-1, thyroid, celiac screen), and depending on results, a growth hormone stimulation test.

Bone age is the most useful early data point most parents never hear about. An "advanced" bone age versus a "delayed" bone age tells you a lot about how much growth is left on the clock.

"The referral I almost did not push for was the one that changed everything."
